If you're an Auckland-based early learning service, Kōhanga Reo or language nest, please use the form below to notify public health of an outbreak at your centre. We can then provide public health support and guidance to help stop the spread of the illness and protect staff and tamariki.

This form should be completed when three or more staff or tamariki from a single area/room of your site have gastroenteritis symptoms (vomiting or diarrhoea).

We will aim to respond to queries within one working day. For urgent queries please call (09) 623 6400 and ask for the Health Protection Officer on duty.
